摘要
This lecture will discuss the current state of endovascular thrombectomy in ischemic stroke and future directions in adjunctive treatment in the post-reperfusion era and pre-symptomatic intervention in high-risk subjects. The issues around adjunctive neural stem cell-based therapy and pharmacological targets of neuroprotection for clinical trials will be discussed, particularly Nerinetide and aryl hydrocarbon receptor modulation. In post-acute patients, enhancement of neuroplasticity utilizing non-invasive transcranial brain stimulation will be outlined. We conducted a randomized controlled trial of bihemispheric tDCS during task-oriented training, which facilitated motor recovery in early subacute stroke patients. Next-generation sequencing technology has challenged pre-symptomatic risk prediction. We unravel a significant proportion of stroke-causing variants that might have been overlooked in a diverse spectrum of familial stroke phenotypes, particularly with small vessel disease, intracerebral hemorrhage and other undetermined infarction. The results reveal the characteristic genetic epidemiology in Taiwan and support the feasibility of advanced genetic diagnosis of well-characterized familial stroke.
