摘要

Parkinson’s disease (PD) is a progressive nervous system disorder that affects movement. The clinical presentation of PD is heterogeneous and overlaps  with  other  conditions,  such  as  the  parkinsonian  variant  of multiple system atrophy (MSA-P), progressive supranuclear palsy (PSP), and essential tremor, which makes accurate diagnosis difficult especially in  the  early  stage.  Brain  imaging  in  patients  with  parkinsonism  may increase  the  accuracy  of  differential  diagnosis  and  thus  help  subject treatment. Moreover, molecular imaging is well suited for the detection of preclinical or prodromal PD cases. Single photon emission computed tomography (SPECT) and Positron emission tomography (PET) provide function  and  molecular  features  in  vivo  in  patients  with  PD.  Tc-99m TRODAT SPECT is available in Taiwan for routine use in clinical practice to   help   clinicians   differential   diagnosis   between   PD   and   nondegenerative  tremors.  In  addition,  PET  imaging  using  FDG  or  AV133 (targeting vesicular monoamine transporter 2) can help classify among PD  and  atypical  parkinsonism  (MSA-P  and  Lewy  Body  Dementia). Recently  novel  tau  PET  image  had  added  more  utility  to  assessing parkinsonian patients. Herein, the presentation provides an overview of molecular  imaging  advances  and  how  these  approaches  help  us  to understand PD and atypical parkinsonisms.
